Generally speaking, this issue stems from a line of CSS on your website. In your CSS file "style.css " , check if there is the rule (input {-webkit-appearance: none;-moz-appearance:none;border-radius:0px;}). If this is the case, you need to remove -webkit-appearance: none;-moz-appearance:none; to put everything back in order.
